719f636004 fix: 将 2018~2020 年项目列表迁移至独立文件，修复 GitHub README 渲染截断问题 
README.md 超出 GitHub ~512KB 渲染限制，导致页面底部内容无法显示。
将 2018~2020 年的 1535 行内容迁移至 pages/README-2018-2020.md，
README.md 从 527KB 缩减至 420KB。

46709bb0ac fix: 修复自动添加项目的两处格式 bug 
1. 双重嵌套链接：用户用 GitHub 富文本编辑器提交时会产生 [[text](url)](url)
   格式，现在用 fix_nested_links() 净化 header_line

2. 缺少空行：在已有同日期 section 时，新条目和原有条目之间只有一个 \n，
   改为先 lstrip('\n') 再显式加 \n\n，确保始终有空行分隔
