tccgen.c:
- make 'struct default_debug' const
- pass TCCState* as parameter to tcc_debug_xxx functions
- always check tcc_state->do_debug before calling functions
- factor out tcc_debug_extern_sym()
- remove formats "%lld"/"%llu" (not reliable on windows)

xxx-gen files:
- set func_vt/var from caller
